Gallardo Health Services is a client-centered organization dedicated to helping individuals and families navigate complex healthcare, social service, and life-transition systems with clarity and confidence. Our Services
Community Choice
The Community Choice Waiver is designed for individuals who: Are 19 years of age or older, or are 65 years of age or older and blind or disabled, and meet nursing facility level of care, but prefer to remain in their own home or an assisted living setting. Gallardo Health Services provides comprehensive case management for participants of the Community Choice Waiver. Our services include: Guidance and support with the Medicaid application process Connecting individuals with needed services and supports in their local community Ongoing oversight of services and overall well-being through monthly or quarterly in-home visits We are committed to helping individuals maintain independence while receiving the care and support they need in the setting they choose.
Developmental Disabilities
The Developmental Disabilities (DD) Waiver offers services and supports to individuals with developmental disabilities and acquired brain injuries. Assistance is provided in the home and throughout the community to help individuals build, maintain, and strengthen independent living skills, increase community involvement, explore new interests, and expand existing hobbies. Gallardo Health Services currently provides case management services through the DD Waiver. Waiver Application
Once you choose a case manager, they will begin providing Targeted Case Management to support you through the waiver application process. This includes help with determining Medicaid eligibility, arranging required psychological evaluations, and completing the ICAP (Inventory for Client and Agency Planning), all of which are necessary to establish waiver eligibility. Resource Support while on the Waitlist While waiting for waiver funding, your case manager can connect you with community programs and local social services to help meet immediate needs. These resources can provide support until funding is approved and a Plan of Care is put in place. Plan of Care Oversight After enrollment in the waiver, your case manager will oversee the implementation of your Plan of Care. This involves completing required home visits and service observations, confirming that services align with your goals and needs, monitoring overall health and safety, and maintaining open communication with providers, family members, and others who are part of your support network.
